I got mine from the Afghan Embassy in London. Fill in form, send passport and cheque and wait (there was a surprising quick turnaround) - I seem to remember there was a requirement for an invitation letter although that might have changed. I've never heard of tourist visa on arrival.

I got mine in Khorog, southern Tajikistan. Just fill out a form, give them a passport photo and itinerary, walk to the bank and pay $41 each I think. They get it done within a few hours if you ask nicely. Normal business hours weekdays and sometimes open on Saturdays. I've heard stories of London being a lot fussier about itineraries and pre-booking accommodation.
