There is a big difference between the sex trade for tourists and village brothels that cater to local Thais. The girls in the villages are more likely to be indentured, and nobody really cares about their health. Their customers are less likely to use condoms... if the girls even know to use condoms in the first place.

Tourists, on the other hand, are well informed of the dangers of HIV.
