So do these things have any effect in a war, or are they just something that keeps the other guys on their toes?

These would be virtually useless except at point blank range. They would likely cause only injury not death and be just as hazardous to the user. To the unskilled gun user (which these dudes obviously are as they load the contraption and then look down the "barrel" to check it) it would be more about putting something in the hands of a rebel to make them feel armed and confident. Likely given to teens or child soldiers. Net effect on the battlefield would be nil but its good for threatening civilians etc. who don't know any better with noise, flash, and smoke.
